WIC First Visit

Please bring the following information to your first Women, Infants, and Children (WIC) visit. If you do not have some of the information on this list, please come to your appointment anyway.

Person Being Enrolled
The pregnant woman, new mother, infant, or child being enrolled must come for the first visit.

Proof of Legal Name
Show proof of legal name for an infant by bringing hospital discharge papers, a nursery bracelet, a birth certificate or a crib card. For a child please bring an immunization record or a birth certificate.  For an adult please bring a photo I.D.

Photo ID
Adults should present a photo I.D., such as a driver's license.

Proof of Missouri Address
Bring a utility bill, telephone bill, voter's registration card or another letter addressed to you.

Proof of Income / Medicaid (MOHealthNet) Card
Bring one month of pay stubs or Social Security payment stub. Your gross income must fall into the WIC program eligibility guidelines. If you have a Medicaid card, please bring a card for each person enrolling in the program.

Child / Infant's Immunization Records
Please bring records of any immunizations received.
